Medical questionnaire
In order to provide you with the best possible care, it is important that we are aware of your medical condition. Your general health may have an impact on your oral health and/or require special precautions during treatment. It is also important for us to know what dental care you have already received. This will enable our team to make a more accurate diagnosis and offer you treatment tailored to your condition. It is therefore essential that we are provided with this valuable information at your first appointment.
That is why we ask you to fill out our medical questionnaire at home and bring it with you on your first visit. This will reduce your waiting time and allow our dental team to prepare adequately.
